Position: Behavioral Health RN Case Manager

Location: Webster/Southbridge Area

Schedule: Full-Time Days – Flexible Hours

Sign-On Bonus: $5,000

What You’ll Do

  • Conduct in-home behavioral health assessments and develop individualized care plans.
  • Provide compassionate, one-on-one patient care focused on both physical and mental well-being.
  • Educate patients and families on health management and coping strategies.
  • Communicate patient progress and any concerns with physicians and the clinical team.
  • Adjust care plans based on patient needs and treatment effectiveness.
  • Mentor and supervise behavioral health aides and LPNs.

Qualifications

  • Active, unrestricted RN license in the state.
  • Minimum of one year of nursing experience in a clinical or behavioral health setting.
  • Ability to travel within the assigned coverage area (50% travel required).
  • Reliable transportation with a valid driver’s license and current auto insurance.
  • Associate or Bachelor’s degree in Nursing (preferred).
  • Prior home health or hospice experience is a plus.
